Összefoglalt célok: 1. a korai tüdőrák diagnosztikájának és kezelésének lehetséges módszerei 2. tüdőrákos esetek kapcsán a posztoperatív kemoterápiára leginkább hatást gyakorló perioperatív faktorok vizsgálata, illetve azon faktorok jelentősége, melyek a legtöbb komplett kemoterápiás ciklus felvételéhez járulnak hozzá (1. Vizsgálat) 3. a nyitott mellkassebészeti módszerek szerepének vizsgálata, többszörös mini-thoracotomia kapcsán végzett multiplex tumor reszekció során (2. Vizsgálat) 4. a kezelőorvos és beteg közötti közös döntési folyamat (shared decison making) jelentősége, annak érdekében, hogy korai tüdőrák eseteiben a legelőnyösebb kezelési módszer kerüljön alkalmazásra 5. myasthenia gravis kapcsán a legjobb neurológiai eredményekkel járó thymectomia meghatározása (3. Vizsgálat) 6. a thymus elváltozásainak tekintetében a jelenlegi diagnosztikai és képalkotó módszerek vizsgálata, valamint az MRI szerepének meghatározása a különböző típusú thymomák, thymus hyperplasia és ektópiás thymus szövet kapcsán 3 Modern diagnosztikus és kezelési lehetőségek tüdőrák esetében 3.1 A korai stádiumú nem kissejtes tüdőrák kezelési lehetőségei A tüdőrák manapság világszerte a rákkal kapcsolatos halálozások vezető oka. Alapvetően két típusa van, a nem kissejtes tüdőrák (NKST) (ide tartozik az adenokarcinóma és a laphámrák) és a kissejtes tüdőrák. A korai NKST kezelése hagyományosan a lobektómia volt, a mediastinalis nyirokcsomók eltávolításával. A sztereotaktikus test besugárzás (SZTB) egy alternatív kezelési lehetőség, mely főként inoperábilis esetekben, jelentős komorbiditással, vagy rossz légzésfunkcióval rendelkező, illetve idős betegek esetében alkalmazható. Az SZTB ígéretes eredményekkel rendelkezik a NKST-ok lokális kontrolljának tekintetében, melyet az RTOG 0236 tanulmány eredményei is megerősítenek, mely szerint alkalmazásakor a 3 éves lokális- és lokoregionális recidíva-mentes túlélési arány 91%, illetve 87% volt. Mitöbb, az SZTB kezelést követő 2 évre vonatkozóan, retrospektív tanulmányok 4,9% és 7,8% lokális- és regionális recidíva arányról számolnak be. A sebészi úton eltávolított nyirokcsomók célja a megfelelő betegcsoport kiválasztása, valamint az esetleges műtéti túlkezelés elkerülése. Előzetes szövettani minták nélkül mindössze a klinikai stádium hasonlítható össze, melynek különleges jelentőssége van a tekintetben, hogy SZTB-t követően a regionális recidívák akár 15%, míg a mediastinumban megjelenők 7,5%-ban is előfordulhatnak. A Society of Thoracic Surgeons, mellkassebészeti társaság célratörően fogalmazza meg ajánlását a korai tüdőrákok kezelésével kapcsolatban, miszerint „a legkevesebb parenchyma reszekcióval járó, az aktuális diagnosztikai- és onkológiai irányelveket betartó sebészeti módszert kell alkalmazni, a legkevésbé invazív behatolásból”. 3.2 A VATS módszer jelentőssége előrehaladott tüdőrák kapcsán- a perioperatív faktorok hatásainak vizsgálata Azzal együtt, hogy az előrehaladott tüdőrákos esetekben a sebészeti módszer szerepe elsődleges, a platina alapú posztoperatív kemoterápia (PAPK) bevezetése szignifikánsan növeli az összesített túlélési arányt. Célunk volt a legfontosabb, posztoperatív kemoterápiás ciklusokat befolyásoló perioperatív faktorok vizsgálata, valamint eredményeink alapján rávilágítani a ciklusok számának lehetőség szerinti növelésére. 3.2.1 Betegek és módszer Vizsgálatunkban 6 év alatt (2011.01.01-2016.12.31) 72, olyan, szövettanilag IB, vagy magasabb stádiumba tartozó (kivéve IV stádium) tüdőrákos beteg adatait vizsgáltuk, akik műtét utáni onkológiai kezelésben részesültek. A következő szempontokat elemeztük: a nyitott- és VATS lobectomiák aránya, műtéti idő, posztoperatív láz, vér transzfúziós igény, reoperációk aránya, hosszantartó levegő átszívások aránya, szövettan, patológiai stádium, nemek aránya, body mass index (BMI), Malnutrition Universal Screening Tool (MUST), Charlson comorbidity index (CCI), forced expiratory volume 1 second (FEV1), anamnézisben lévő korábbi malignus betegségek aránya, pitvarfibrilláció, performance status (PS). 3.2.2 Eredmények A 72 betegre vonatkoztatva (T (teljes): n= 53 [73,61%]; NT (nem teljes): n= 19 [26,38%]) az átlag életkor valamivel magasabb volt a T csoportban (64,11 év), azonban a nemek aránya, FEV1, BML, MUST, az anamnézisben lévő korábbi malignus betegségek aránya, pitvarfibrillációk arány, valamint a PS tekintetében a két csoportra vonatkoztatva szignifikáns különbséget nem találtunk. Nem találtunk továbbá szignifikáns eltérést a posztoperatív láz, vértranszfúziós igény, hosszantartó levegő átszívás, reoperációk, szövettani megoszlás (adenokarcinóma vs laphámrák), illetve pathológiai stádiumok tekintetében sem. Egyváltozós regressziós analízis kapcsán jelentős, azonban nem szignifikáns különbséget találtunk a két csoportra vonatkoztatva a VATS javára...

Absztrakt (kivonat) idegen nyelven

1 Introduction The surgical methods of accessing the inner thoracic organs has evolved throughout recent decades. After the introduction of the minimal access surgical approach in the 1980s, it was rapidly acquired for thoracic procedures, leading to the development of the so called Video-Assisted Thoracic Surgery (VATS) approach. The advantages of VATS- including reduced postoperative wound pain, minimized intraoperative blood loss, shorter hospital stay and improved postoperative quality of life- have gradually come in the limelight, with great acceptance from surgeons and patients alike. Features of VATS not only include better cosmesis with smaller incisions, but also decreases systemic inflammatory response caused by general anesthesia and intubation. During our thesis our purpose was to address questions and debates concerning VATS and define its place in the modern treatment of early stage (ES)- and advanced stage (AS) LC, with special regards to thoracotomy and alternative treatment options such as stereotactic body radiation therapy (SBRT). In the modern era of individualized patient care, our work also emphasizes the need for more patient tailored treatment discussion and the need for shared decision making (SDM) between physicians and patients. Despite this growing body of evidence favouring the VATS approach, the question of debate remains regarding the role and place of thoracotomies and their value, with the obvious question flashing: is traditional thoracotomy considered obsolate in the emerging era of VATS? The VATS approach not only applies for the treatment of lung cancer, but also plays a major role in the surgery of the mediastinum, especially in the surgery of the thymus. However numerous questions arise in connection with the minimal access approach. Is the minimal access approach as good as traditional sternotomy and is it capable of reaching R0 resection and complete removal of ectopic thymic tissue (ETT)? When so, is it accompanied by better surgical and neurological results in myasthenia gravis (MG), even with facilitation of cosmetic outcome? In addition, what are the perioperative rates of morbidity and mortality in each method, and how does the chosen surgical technique affect long term quality of life and rates of remission? In terms of thymic imaging, should diagnostic imaging of the thymus stick with conventional CT, or can MRI also be helpful in differentiating between thymic abnormalities? Can MRI be able to more accurately point out ectopic thymic foci and thus lead the scalpel? 2 Objectives Addressed questions summarized: 1. to review best diagnosis and treatment options for early stage lung cancer 2. study on the effect of perioperative factors influencing postoperative chemotherapy treatment and deciding which factors have the most positive influence in receiving the highest number of complete postoperative chemotherapy cycles (Study 1) 3. defining the role of open thoracic procedures in the current minimal access era through the presentation of multiple tumor resections via mini-thoracotomy (Study 2) 4. to emphasize the need for shared decision making between physicians and patients, in order to choose the best treatment option for early stage lung cancer 5. defining the best surgical treatment options with early neurological results of thymectomies in patients with myasthenia gravis (Study 3) 6. to review current diagnostic and imaging options in various thymic conditions and define the place of MR imaging of thymomas (and subtypes), thymic hyperplasia and ectopic thymic foci 3 Modern diagnostic and treatment options for lung cancer 3.1 Treatment approaches in the treatment of early stage NSCLC Lung cancer is the leading cause of cancer-related deaths worldwide. Lung cancer is divided into non-small cell lung cancer (NSCLC) and small cell lung cancer (SCLC), the former of which includes adenocarcinoma and squamous cell carcinoma. Recommended treatment for ES-NSCLC has historically been lobectomy with mediastinal lymph node dissection. Stereotactic body radiation therapy has been used as an alternative treatment therapy for patients with inoperable lung cancer and in patients considered to be at high-risk as a result of comorbidities, poor pulmonary function, and/or advanced age. Stereotactic body radiation therapy remains a promising modality for local control of NSCLC which is demonstrated by the 91% and 87% 3-year local and loco-regional recurrence free survival rates observed in RTOG 0236 study. Moreover 2 years after SBRT treatment, a 4.9% and 7.8% local- and regional recurrence rate was noted in retrospective studies...
